Our Seamless Referral Process

At Vivo Infusion, we prioritize efficiency, communication, and patient-centered care. Our referral process ensures a smooth transition for both physicians and patients, with dedicated support every step of the way.

Step 1: Referral Submission

  • The patient’s physician downloads and completes the referral form and submits it via email or fax with clinical notes, demographics, and an insurance card.
  • A Patient Care Navigator confirms receipt with the physician’s office via a referral notification.

Step 2: Benefit & Referral Assessment

  • Our team reviews the referral packet for completeness.
  • Our insurance specialists contact the patient’s health plan for benefit verification and prior authorization if required.
  • If additional financial documentation is needed, a Patient Resource Specialist will reach out to the patient.
  • A Patient Care Navigator welcomes the patient and answers any initial questions.

Step 3: Infusion is Scheduled

  • Once benefits are confirmed, our team contacts the patient to schedule their infusion treatment.
  • New patients receive a “Center Introduction” to provide logistical details and answer any concerns.
  • The Patient Resource Specialist reviews insurance benefits and assistance programs to prepare for the patient’s first appointment.
  • After the patient’s infusion, a treatment record is sent to the referring physician.

Step 4: Follow-Up Care

  • A post-infusion summary is emailed to the referring office, including progress and treatment notes.
  • A Patient Care Navigator remains available at 720.902.4111 to assist with any questions.
  • One of our local Infusion Specialists follows up with the referring office to ensure the best possible infusion experience.
